“Most women seeking a breast reduction have breasts that are significantly out of proportion to the rest of their body,” says Dr. Kobienia. “In most cases, breast reduction is performed on the fully mature breast – or after a woman is at least 18 years of age. There is no age criteria for a breast reduction, however the breasts should be a stable size for at least a year prior to considering a breast reduction. It’s important to note that younger women in their teens and twenties who have breast reduction surgery may be at increased risk for having a recurrence of macromastia/large breasts in the future.”
How is Breast Reduction Surgery Performed?
Breast reduction surgery is typically performed under general anesthesia and takes approximately 2-4 hours, depending on the complexity of the procedure. The surgical techniques used may vary based on a patient’s anatomy, goals, and their surgeon’s recommendations.
The most common methods include:
Anchor or Inverted-T Incision – This method involves three incisions: one around the areola, one running vertically down to the breast crease, and one along the breast crease. It allows for significant tissue removal and reshaping, making it ideal for patients with extremely large breasts.
Lollipop Incision (Vertical Scar Technique) – This technique involves an incision around the areola and a vertical incision down the center of the breast. It is best for moderate breast reductions and provides a more lifted appearance.
Liposuction-Only Reduction – In some cases, liposuction alone may be used to remove excess fat from the breasts, particularly for patients who need only a minor reduction and have good skin elasticity.
“During breast reduction surgery, excess glandular tissue, fat, and skin are removed, and the remaining breast tissue is reshaped for a more proportionate appearance,” says Dr. Kobienia. “The nipple and areola may also be repositioned or resized to maintain a natural look. Liposuction is often used to remove excess fat from the armpit area and allow for a smooth transition between the underarms and breasts.”
Recovery Process After Breast Reduction Surgery
The recovery period after breast reduction surgery is crucial for optimal healing and results. Your surgeon will use scar reduction techniques like careful suturing, tape support, and compression garments. Scars will fade in color and intensity over time – generally one to two years. You should know, however, that breast reduction is a trade of large and heavy breasts for a more manageable size and shape with incisions and subsequent scars.

- After surgery, you will be placed in an elastic bandage and gauze, or perhaps a post-surgical bra over gauze dressings.

- You will return for a follow-up exam in one to three days.
